Intake Coordinator

The Intake Coordinator is a vital member of the Compassus team, responsible for modeling the company's values and promoting the Compassus philosophy. This role requires strong communication and organizational skills, with a focus on coordinating patient care and ensuring seamless transitions between departments.
Key Responsibilities
- Assist with completing Flexport templates and creating charts in Home Care Home Base (HCHB) based on referral information.
- Respond promptly and courteously to patient and internal customer inquiries or problems.
- Coordinate with Admissions Coordinators to process received referrals and complete coordination notes for pending charts.
- Manage the non-admit folder in Forcura and maintain the Home Health Intake Issue Tracker to assist other departments in resolving patient admission issues.
- Collaborate with referral providers and/or growth team to obtain missing information needed to complete referrals.
- Ensure all referral documentation is uploaded in a timely manner to the Home Care Home Base (HCHB) record for insurance verification.
- High school diploma or GED required.
- One (1) or more years of experience in a healthcare office setting required.
- Mathematical skills: ability to add, subtract, multiply, and divide in all units of measure.
- Language skills: ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations.
- Strong written and verbal communication skills.
The demands of this role necessitate a team member to effectively perform essential functions. Adaptations can be made to accommodate team members with disabilities. Regular standing, walking, and manual dexterity are fundamental, along with the ability to lift and move objects up to 25 pounds. Visual acuity requirements include close and distance vision, color and peripheral vision, depth perception, and the ability to adjust focus.
