Flexible Home-Based Physical Therapist Opportunity

Mass General Brigham is currently seeking a skilled and compassionate physical therapist to join our team in providing home-based care to patients in the Chelsea, Massachusetts area.
About the RoleThis per diem position offers the opportunity to work with a diverse patient population, providing acute-level care in the comfort of their own homes. As a key member of our clinical team, you will be responsible for developing personalized plans of care that focus on patient-centered goals and outcomes.
You will conduct virtual and in-person evaluations, as needed, and apply standardized interventions to promote optimal patient outcomes. Your daily documentation will be accurate and timely, ensuring seamless communication with the full clinical care team.
Key Responsibilities- Develop patient-focused plans of care, incorporating standardized interventions to achieve goal-oriented outcomes
- Conduct virtual and in-person evaluations, as required, to assess patient needs and progress
- Daily documentation must be accurate, timely, and transparent to support real-time updates visible to the full clinical care team
- Apply evidence-based treatment procedures to ensure effective patient outcomes
- Provide age-appropriate education to patients and caregivers, promoting self-management and empowerment
The ideal candidate will possess excellent communication and interpersonal skills, with the ability to adapt to changing situations and priorities. You will be expected to maintain up-to-date knowledge of industry trends and best practices, applying this expertise to enhance patient care and outcomes.
Requirements and Qualifications- Registered Physical Therapist (PT) licensure in Massachusetts
- Minimum of two years' experience in homecare and/or acute care settings, preferably with Epic and Rover experience
- Strong customer service skills, with the ability to work effectively with diverse populations
- Excellent verbal, written, and electronic communication skills
- Computer proficiency, with the ability to learn new software applications
- Ability to prioritize tasks, manage time efficiently, and adapt to changing situations
