About the Role:

We are looking for a highly motivated and organized individual to coordinate patient care services in various settings within the community. The ideal candidate will have experience in healthcare, excellent communication skills, and the ability to work effectively with patients, families, and interdisciplinary teams.

Key Responsibilities:

  • Develop and implement personalized care plans that meet the unique needs of each patient
  • Coordinate with healthcare providers, therapists, and other specialists to ensure seamless care delivery
  • Communicate effectively with patients, families, and caregivers to promote understanding and adherence to care plans
  • Maintain accurate and up-to-date records of patient progress and care activities
  • Collaborate with the care team to identify and address any challenges or concerns

Requirements:

  • Bachelor's degree in nursing or related field (or equivalent experience)
  • Minimum 1 year of experience in healthcare coordination or a related role
  • Strong organizational and time management skills
  • Excellent communication and interpersonal skills
  • Able to work independently and as part of a team

Salary:

The estimated annual salary for this position is $83,500-$101,000 based on location and experience.
