About the Role

We are seeking a highly skilled and compassionate RN Case Manager to join our team at Heart 'n Home Hospice, part of LHC Group. As an end-of-life care expert, you will have the opportunity to develop trusting relationships with patients and their families, providing them with dignity and comfort during their most vulnerable time.

Responsibilities
  • Conduct initial nursing evaluations to determine eligibility for hospice services within 48 hours of referral.
  • Assess patients' physical, psychosocial, and environmental needs, re-evaluating as needed every 15 days.
  • Develop and implement personalized plans of care, maintaining accurate clinical notes and documenting patient progress.
  • Collaborate with patients, families, attending physicians, and other healthcare professionals to provide comprehensive care.
Requirements
  • Current RN licensure in the state of practice and one year of clinical experience.
  • Current CPR certification and valid driver's license with vehicle insurance and access to a dependable vehicle or public transportation.
Compensation and Benefits

The estimated salary for this role is $65,000 - $85,000 per year, depending on experience and location. We offer a competitive benefits package, including employee-focused wellness and support programs, to ensure your well-being and success in this critical role.
