Geriatric Care Specialist

Job OverviewMiracle Mile Healthcare Center is a leading provider of skilled nursing, assisted living, and senior living communities. We are committed to delivering high-quality care that meets the unique needs of each resident.
The successful candidate will have previous experience working with geriatric populations or in long-term care settings. A high school diploma or equivalent is required, although an associate's or bachelor's degree is preferred.
Key Responsibilities- Assist in interviewing and obtaining information from residents and families to develop psychosocial histories, assessments, and care plans.
- Support residents and their families during transitions and adjustments to placement in our facility.
- Determine the legal status of residents upon admission, provide information, and obtain copies of advance directives.
- Inform residents and their families about personal and property rights.
- Provide input on social, psychological, and emotional needs of residents and their families to supervisors.
- Ensure residents receive necessary evaluations for visual, audio, mental health, and dental needs.
- Arrange appointments and transportation for residents as needed.
We offer competitive pay ranging from $22.00 to $25.00 per hour, dependent on factors such as years of experience, education, and industry background. Full-time employees are eligible for comprehensive medical, dental, and vision insurance plans, tuition assistance, and 401K benefits.
Requirements- Previous experience working with geriatric populations or in long-term care settings.
- High school diploma or equivalent; associate's or bachelor's degree preferred.
- Excellent communication and interpersonal skills.
- Able to interact tactfully, effectively, and professionally with residents, families, and staff.
- Ability to work independently and as part of a team.
- Sufficient computer skills, including proficiency in Microsoft Office and Outlook.
