About the Role

Abode Care Partners is seeking a compassionate Medical Care Specialist to join our team. As a licensed Nurse Practitioner, you will deliver high-quality medical care to seniors and patients with complex needs.

This role involves diagnosing and treating various illnesses, ordering and interpreting diagnostic tests, and collaborating with healthcare professionals to maintain comprehensive care plans. You will have the opportunity to enhance your medical skills while making a significant impact in patients' lives.

Responsibilities
  • Evidence-Based Medical Care: Plan, direct, and evaluate patient medical needs.
  • Chronic Illness Management: Examine and treat chronic and urgent episodic illnesses, minor injuries, accidents, and other injuries or illnesses.
  • Clinical Assessment: Order and interpret laboratory and diagnostic testing as needed.
  • Interdisciplinary Care: Collaborate with caregivers and other healthcare professionals in providing comprehensive care and maintaining a plan of care.
  • Patient Referrals: Provide referrals to other healthcare practitioners when necessary.
Qualifications
  • Licensure: Active state Nurse Practitioners license.
  • Certification: Current Nurse Practitioner National Board Certification.
  • Education: Master's degree in Nursing (MSN).
  • Experience: Five or more years of nursing experience; at least two years of nurse practitioner experience preferred.
  • Skills: Strong interpersonal communication skills – verbal and written, Excellent time management skills, Proficiency in medical instruments and equipment.
