RN - Skilled Nursing Care Specialist

As a skilled nursing care specialist at SSM Health, you will be responsible for providing high-quality care to residents with acute, subacute, and chronic conditions. This role requires a strong nursing background and the ability to work collaboratively with a team to ensure the best possible outcomes for our patients.
Key Responsibilities- Provide direct patient care, including administering medications and treatments, and maintaining accurate records.
- Collaborate with the interdisciplinary team to develop and implement individualized care plans.
- Conduct regular assessments and evaluations to ensure the highest level of care.
- Communicate effectively with residents, families, and healthcare teams to ensure seamless care transitions.
- Participate in quality improvement initiatives to enhance patient outcomes and satisfaction.
- Graduate of an accredited nursing program.
- Current RN licensure in the state of practice.
- Basic Life Support (BLS) certification.
- Ability to work in a fast-paced environment and prioritize tasks effectively.
- Excellent communication and interpersonal skills.
