LPN Case Manager

As a Licensed Practical Nurse (LPN) in our Case Management team, you will play a vital role in providing skilled nursing services to individuals with mental health, substance abuse, or developmental disabilities. Under the direction of a Registered Nurse or medical doctor, you will assess, plan, and implement individualized care plans to promote a safe and therapeutic environment.
Key Responsibilities:
- Provide skilled nursing services to clients, including assessment, treatment planning, and direct service provision.
- Manage client risk and safety, utilizing specialized knowledge and skills.
- Collaborate with interdisciplinary teams to develop and implement person-centered care plans.
- Travel locally to clients' homes, shelters, and other locations as necessary.
- Participate in state or federally funded programs, such as Medicaid.
Requirements:
- Graduate of an accredited nursing school.
- Valid license as a Licensed Practical Nurse in the State of Florida.
- Two years' experience in a behavioral health setting.
- Certified in CPR.
- Ability to effectively perform client/patient behavior management techniques (CPI) with appropriate training.
- Eligibility to participate in state or federally funded programs.
