Job DescriptionAs a Clinical Care Specialist, you will play a vital role in delivering high-quality resident care. Your responsibilities will include:
- Providing direct resident care, including administering medications and performing various clinical tasks
- Conducting thorough assessments and developing individualized care plans
- Collaborating with other healthcare professionals to ensure seamless care delivery
- Maintaining accurate and detailed records of resident care

Required Skills and QualificationsTo be considered for this position, you must:
- Hold a current and valid LPN nursing license for the state of Illinois
