Responsibilities- Conduct comprehensive assessments of patients' physical, emotional, and socio-economic needs.
- Develop and implement individualized plans of care in collaboration with the healthcare team.
- Provide education and support to patients and their families on health-related topics, including nutrition, safety, and wellness.
- Collaborate with physicians and other healthcare professionals to ensure seamless care coordination.
- Monitor patients' progress, report any changes or concerns to the healthcare team, and make adjustments to care plans as needed.
- Participate in quality improvement initiatives and contribute to the development of policies and procedures.
- Meet all compliance requirements and maintain accurate records.
- Minimum of zero to six months of experience in a qualified home health agency.
- Nursing certification and BSN degree preferred.
- Excellent communication and interpersonal skills.
- Ability to work independently and as part of a team.
