Job Title: Home Health Aide

Trinity Health At Home is seeking a compassionate and skilled Home Health Aide to provide high-quality care to our patients in their homes.

Job Summary:

We are looking for a dedicated and caring individual to join our team as a Home Health Aide. As a Home Health Aide, you will provide personal care and support to our patients, assisting with daily living activities, such as bathing, dressing, and grooming. You will also assist with household tasks, including meal preparation and light housekeeping.

Responsibilities:
  • Provide personal care and support to patients, including bathing, dressing, and grooming.
  • Assist patients with daily living activities, such as eating, toileting, and transferring.
  • Assist with household tasks, including meal preparation and light housekeeping.
  • Perform range of motion and other simple procedures as assigned by a registered nurse.
  • Document patient care and progress in the clinical record.
Requirements:
  • Completion of a federally compliant training program as required by law/regulation.
  • Successful completion of a competency evaluation program.
  • One year's employment as an aide in an institution, home care, or hospice setting.
  • Ability to function in a practice environment with minimal direct supervision.
