Job Summary

As a Licensed Practical Nurse - LPN at Fairhope Health and Rehabilitation, you will be responsible for providing direct care to residents as needed in accordance with the care plan, policies, and procedures. Your primary focus will be on delivering high-quality patient care, ensuring the well-being and safety of our residents.

Key Responsibilities
  1. Admit, transfer, and discharge residents as required, and inform staff accordingly.
  2. Regularly check residents to observe and evaluate their physical and emotional status.
  3. Direct Nursing/Resident Care Assistants in their responsibilities and report any concerns to the Resident Care Coordinator.
  4. Document care provided to residents in accordance with policies and procedures.
  5. Chart nurse's notes in an informative, descriptive, and legible manner.
  6. Report discrepancies, problems, and issues regarding resident care to the Resident Care Coordinator.
  7. Transcribe and/or check physician orders to ensure they have been executed.
  8. Administer and document medication administration per physician's orders.
  9. Complete assessments on all assigned residents on each shift.
  10. Report residents' condition and shift occurrences to oncoming staff.
  11. Communicate with physicians, residents, families, and nursing staff regarding pertinent information and clinical data.
  12. Provide direct care to residents as needed in accordance with the care plan, policies, and procedures.
Requirements

Currently licensed as a Licensed Practical Nurse - LPN. One (1) year experience in a healthcare facility preferred. Must possess CPR certification within thirty (30) days of employment.
