Nurse Care Specialist

Aveanna Healthcare is a leading national provider of home health services to medically fragile children and adults. We are seeking an experienced Nurse Care Specialist to join our team in providing high-quality care to patients in their homes.
Job Description:
- We are currently looking for a dedicated RN/LPN/LVN to work with a medically fragile patient in the school setting, Monday through Friday from 9am to 5pm.
- The ideal candidate will have six months prior hands-on nursing experience and possess reliable transportation.
Responsibilities:
- Implement the physician's prescribed plan of care for our patients.
- Conduct patient assessments and coordinate care to ensure the highest level of quality care.
- Administer prescribed medication, treatments, and therapies as ordered by the physician.
- Compliantly document all care provided in our system of record.
- Teach and train family members on health promotion and disease management.
- Accept and write verbal orders from physicians as required, obtaining co-signature from the Case Manager.
- Follow up with physicians, clients, family members, staff, and Case Managers regarding client problems, concerns, or changes in client condition.
- Perform other duties as assigned by management.

Qualifications:
- Must have an active, unencumbered license (RN/LPN/LVN) in the state where you'll practice.
- Current CPR certification from American Red Cross, American Heart Association, or BLS.
- TB skin test (current within last 12 months).
- Six months prior hands-on nursing experience preferred but not required.
- Must have reliable transportation.
