Job Overview

Sunrise Senior Living is seeking a dedicated and compassionate Wellness Nurse to join our team. As a key member of our community, you will play a vital role in ensuring the overall well-being of our residents.

Responsibilities
  • Conduct regular wellness visits for all residents to identify on-going needs and services.
  • Collaborate with the Resident Care Director to ensure proper follow-through and documentation of resident care.
  • Assist with resident evaluations and data gathering to inform care decisions.
  • Monitor and respond to resident incidents, documenting and reporting as required.
  • Communicate effectively with residents, families, and healthcare providers to ensure seamless care coordination.
Qualifications
  • Graduate of an approved college or school of nursing.
  • Current state license as a Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN).
  • Minimum of one year experience in a healthcare setting, preferably in senior living or long-term care.
  • Strong assessment and communication skills, with the ability to work independently and as part of a team.
