About the Role:

The Registered Nurse is responsible for delivering compassionate care to patients and their families. Ensuring timely and adequate delivery of hospice services is essential.

Responsibilities:

  • Assess patient and family needs during home visits and document interventions.
  • Collaborate with the interdisciplinary team in developing patient care plans and documenting outcomes.
  • Ensure continuity of care between patient, team members, and healthcare providers.
  • Communicate patient needs and care plans through the interdisciplinary process.
  • Provide continuing education on hospice care to patients and families.
  • Supervise delivery of patient care and ensure compliance with established plans of care.
  • Participate in on-call rotation and ongoing monitoring of patient appropriateness for hospice services.
  • Maintain compliance with Medicare Conditions of Participation and Joint Commission standards.
  • Support community education and quality improvement activities.

Qualifications:

Requirements:

  • Current license as a Registered Nurse in the State of Alabama
  • Valid CPR
  • Current, valid Alabama drivers license
