Key Responsibilities:
The Experieced LPN for Homecare Position requires a highly motivated and compassionate individual who is committed to delivering exceptional patient care. As a key member of our team, you will work closely with patients and their families to develop and implement personalized care plans that meet their unique needs and goals. Your responsibilities will include:

  • Providing skilled nursing care to patients in their homes, including wound care, medication management, and vital sign monitoring
  • Collaborating with interdisciplinary teams to develop and implement comprehensive care plans
  • Communicating effectively with patients, families, and healthcare professionals to ensure seamless care coordination
  • Maintaining accurate and detailed records of patient interactions and outcomes

Requirements:
To succeed in this role, you will need a valid LPN license, at least one year of experience in pediatric home care, and excellent communication and interpersonal skills. We also require you to pass a background check. We offer a competitive salary range of $25-$35 per hour, depending on experience, as well as excellent benefits, including up to 11 days of paid vacation/sick time, 401(k) retirement plan, employer-paid life insurance, dental and vision insurance, and more.
