Renal Nutrition Specialist

We are seeking a highly skilled Renal Nutrition Specialist to join our team at Renviva Management Services, LLC.

The ideal candidate will possess a deep understanding of renal nutrition and be able to provide expert guidance to patients with Chronic Kidney Disease (CKD) and End Stage Renal Disease (ESRD). This role is responsible for conducting initial nutrition assessments, collaborating with physicians to determine diet prescriptions, and educating patients and families on nutrition needs and meal planning.

• Support the facility's mission, goals, and values by providing exceptional patient care and education.
• Conduct initial nutrition assessments on all newly admitted ESRD patients and follow up as needed.
• Collaborate with physicians to determine nutrition needs and diet prescriptions for CKD and ESRD patients.
• Monitor patient progress, including monthly review of blood chemistries, fluid status, and nutritional status, with interventions as necessary.
• Educate patients and families on nutrition needs, meal planning, and community resources.
• Participate in interdisciplinary facility patient team meetings and contribute to quality improvement initiatives.

• Registered Dietitian as per Commission on Dietetic Registration.
• Dietician State licensure (LD).
• One year of experience in clinical nutrition as a registered dietitian; computer experience preferred.
