Overview

We are seeking a compassionate and skilled Registered Nurse Case Manager to join our hospice team in San Antonio, TX.

Job Summary

The ideal candidate will possess excellent communication and interpersonal skills, with the ability to develop trusting relationships with patients and their families. As a Hospice RN Case Manager, you will assume full nursing responsibility for delivering high-quality care to patients, while continuously evaluating your own performance and making necessary changes to improve productivity and quality of care.

Responsibilities
  • Makes initial nursing evaluations to determine eligibility for hospice services within 48 hours of referral.
  • Identifies patients' physical, psychosocial, and environmental needs, reassessing as needed every 15 days.
  • Initiates and coordinates patient plans of care, maintaining accurate clinical notes on patient condition.
  • Collaborates with patients, families, attending physicians, and other members of the IDG to provide daily care.
Requirements
  • Current RN licensure in the state of practice and one year of clinical experience.
  • Current CPR certification.
  • Valid driver's license, vehicle insurance, and access to reliable transportation.
Salary and Benefits

We offer a competitive salary range of $65,000 - $85,000 per year, depending on experience, plus benefits including health insurance, retirement plan, and paid time off.
