Nurse Practitioner II

We are seeking a highly skilled and experienced Nurse Practitioner II to join our team in the Department of Medicine, Hematology/Oncology Division at the University of California, Irvine Medical Center. As a key member of our interdisciplinary team, you will provide comprehensive care to patients in the inpatient settings, working closely with physicians, nurses, and other healthcare professionals to ensure the highest level of patient care.
Responsibilities- Provide multi-disciplinary care coordination, treatment, education, monitoring, and communication with patients and referring physicians.
- Manage chronic stable medical conditions, routine care, acute or critical care, and medical and surgical procedures per approved standardized procedure.
- Develop and implement individualized care plans, taking into account patient needs, preferences, and goals.
- Collaborate with healthcare teams to ensure seamless transitions of care and optimal patient outcomes.
- Participate in quality improvement initiatives and contribute to the development of policies and procedures.
- Current, active, unrestricted California Registered Nurse license.
- Current, active, unrestricted California Nurse Practitioner license.
- Current, active, unrestricted Furnishing license and DEA registration.
- Active National Provider Identifier (NPI).
- Oncology Nursing Society (ONS) Certification (ACONP) and/or ONS Chemotherapy Certification (obtained within 3 months of hire).
- Ability to establish and maintain effective working relationships across the program and UCI Health System.
- Excellent written and verbal communication skills.
- Ability to maintain a work pace appropriate to the workload.
- Experience with clinical trial patient participation.
