Job Summary

We are seeking a skilled RN Case Manager to join our interdisciplinary team. The successful candidate will be responsible for providing patient and family centered care, conducting assessments, creating and updating plans of care, and collaborating with the healthcare team to ensure high-quality patient outcomes.

Key Responsibilities
  • Conduct patient and family assessments to identify physical, emotional, and spiritual needs
  • Develop and implement individualized plans of care in collaboration with the healthcare team
  • Provide patient and family education on pain management, end-of-life care, and other relevant topics
  • Collaborate with the healthcare team to ensure seamless care transitions and effective communication
  • Participate in meetings to plan and evaluate patient/family services
Requirements
  • ADN or BSN required
  • Current, unrestricted Registered Nursing License in state of employment
  • One-year nursing experience, preferred
  • Valid driver's license, auto insurance, and access to a reliable vehicle
  • Ability to develop, maintain, and apply knowledge of hospice and end-of-life process
  • Ability to maintain professional boundaries with patients, families, and co-workers
