Job Title: Registered Nurse - Home Health

VNA Health Group is seeking a skilled and compassionate Registered Nurse to join our team as a Home Health Care Professional. As a key member of our interdisciplinary team, you will provide high-quality, patient-centered care to individuals in their homes, promoting independence, dignity, and comfort.

Job Summary:

The Registered Nurse will be responsible for assessing patients' needs, developing and implementing individualized care plans, and providing education and support to patients and their families. You will work collaboratively with our interdisciplinary team to ensure seamless care coordination and optimal patient outcomes.

Responsibilities:
  • Conduct comprehensive assessments of patients' physical, emotional, and social needs.
  • Develop and implement individualized care plans that address patients' needs and goals.
  • Provide skilled nursing care, including wound care, medication management, and vital sign monitoring.
  • Collaborate with patients, families, and healthcare providers to ensure comprehensive care coordination.
  • Provide education and support to patients and their families on disease management, self-care, and health promotion.
  • Document patient progress and outcomes in accordance with VNA Health Group's policies and procedures.
Requirements:
  • Current RN licensure in the State of New Jersey.
  • At least 1 year of experience in home health care or hospice care.
  • Ability to work independently and as part of a team.
  • Excellent communication and interpersonal skills.
  • Ability to maintain confidentiality and handle sensitive information.
