Job Summary

We are seeking a skilled Therapist to join our team at Social Work PRN. The ideal candidate will have experience in counseling or substance abuse and be fully licensed or have an associate license.

Responsibilities

The successful candidate will be responsible for providing therapy for Consumers on the ACT Team, assessing and completing 1013 when required, and completing Initial and Comprehensive Assessments. Additionally, they will assist with referrals, complete Treatment Plans when needed, and participate in ACT Fidelity Review, ASO Audit, and CARF Review.

Requirements

Applicants must have a Master's degree in social work and experience in the field of counseling or substance abuse. A fully licensed or associate licensed LCSW or LMSW is required. This is a full-time position, Monday through Friday, 8am to 4pm, with the possibility of temp to perm or permanent placement.
