About the Role

We are seeking a skilled Clinical Nurse Practitioner or Physician Assistant to join our team at Matrix Medical Network. As a Home Risk Assessment Healthcare Professional, you will play a critical role in reviewing and observing patients' current health, medical history, medication adherence, social environment, and other risks during home visits.

This position offers a unique opportunity to make a direct impact on the health and well-being of others while leveraging your expertise and compassion. You will work collaboratively with physicians, case managers, social workers, family members, and other healthcare professionals to provide comprehensive care and support to patients.

Key Responsibilities:

  • Conduct adult and geriatric assessments, medical history, diagnosis, and treatment, health education, physician referrals, case management referrals, follow-up, and clear documentation according to Matrix guidelines and protocols.
  • Work collaboratively with physicians, case managers, social workers, family members, key caregivers, and other ancillary medical personnel as appropriate.
  • Collaborate with Primary Care Physicians (PCPs) on patient education and provide follow-up care.
  • Provide services in a variety of venues, including home visits, skilled facility visits, and telehealth visits.

Requirements:

  • Master's degree required or commensurate experience and satisfactory completion of NP licensure.
  • Current RN and NP or PA licensure in the state of practice, including prescription authority or the ability to obtain prescriptive authority.
  • Board certification by the NCCPA, AANP, ANCC, or the AACCN in a Matrix-approved specialty.
  • Have a current BLS, ACLS, or CPR certification.
  • Have a minimum of 1 year of experience as a Nurse Practitioner/Physician Assistant and the ability to travel in-state or out-of-state.
  • Have strong computer skills and familiarity with EMR software.
  • Possess excellent verbal and written communication skills with patients, clients, and colleagues.
  • Have a positive and outgoing personality and be a team player, comfortable and flexible with frequent change.

Travel Requirements:

  • Have the ability to travel, a valid state driver's license, and be able to drive a car, with proof of adequate automobile insurance coverage for the state of residence.
  • Travel required to meet patients where they live.
