Job Summary

We are seeking a highly skilled RN Case Manager to join our team in Reno County, Kansas. As a Field-Based RN Case Manager, you will be responsible for assessing, planning, and implementing care strategies for Medicare and Medicaid members. You will work closely with members to manage their health and healthcare, utilizing strong communication and interpersonal skills.

Key Responsibilities
  • Assess and plan care strategies for members with complex medical and behavioral needs
  • Implement care plans and provide education to members on self-management of their health and healthcare
  • Collaborate with interdisciplinary teams to ensure quality coordinated care and services are provided to members
  • Communicate with stakeholders to ensure required health-related information is shared
  • Advocate for members to ensure access to care
  • Provide clinical support to non-clinical staff on medications, diagnosis, and disease-specific education
Requirements
  • Current, unrestricted RN license in the state of Kansas
  • 2+ years of experience working in the community as a RN in acute inpatient care, long-term care, home health, hospice, public health, behavioral health, or assisted living
  • Intermediate level of proficiency working with MS Word, Excel, and Microsoft Outlook
  • Ability to travel up to 75% of the time with reliable transportation, with a current and non-restricted state of Kansas Driver's License and State-required insurance
  • Access to a designated quiet workspace in your home (separated from non-workspace areas) with the ability to secure Protected Health Information (PHI)
  • Live in a location that can receive a UnitedHealth Group approved high-speed internet connection or leverage an existing high-speed internet service
Preferred Qualifications
  • Bachelor's degree in nursing
  • A background in managing Medicaid and Medicare populations with complex medical and behavioral needs
  • Experience with arranging community resources
  • Experience with electronic charting
