Developmental Disability Support Coordinator

We are seeking a highly skilled and compassionate individual to join our team as a Developmental Disability Support Coordinator. This is an exciting opportunity to make a meaningful difference in the lives of individuals with developmental disabilities and their families.
About the Role:
- As a Developmental Disability Support Coordinator, you will provide intake and ongoing assessments of infants and toddlers eligible for Early Intervention Services under California Early Start program mandates.
- You will assist eligible children and their families with the coordination of other resources, in the development of an Individualized Family Service Plan, participate in periodic reviews of the IFSP every six months or more frequently if conditions warrant, and take an active role in transition social services for toddlers.
- You will provide social service coordination to assist families in accessing services necessary to enhance the development of the child and the capacity of the family to meet the needs of the child.
- May participate in educational and prevention outreach activities and serve as liaison to other community organizations that work with populations at high risk for having a developmental disability.
Requirements:
- A Master's Degree in Social Work or a related field from an accredited university, or a credentialed Speech Pathologist or Nurse with a PHN or PNP certification, and three years of experience in working with children in early intervention agencies and resources.
- Bachelor's Degree and three years experience in a professional position at NBRC or another California Regional Center with a child case carrying responsibility can be substituted for the above. Minimum education requirement may not be waived.
- Knowledge of the Spanish Language is desirable. Minimum qualifications may be waived depending on the agency's need for specialized skills (such as language abilities).
