The estimated annual salary for this position is around $85,000 - $110,000, depending on experience and qualifications. Job Description

The Primary Care Nurse Program Coordinator plays a critical role in the coordination of nursing care, including direct patient care, patient/family education, and transitions of care. As an advanced nurse professional, you will be accountable for planning, implementing, evaluating, and communicating all phases of nursing care for assigned patients.

Responsibilities
  • C-coordinate nursing care to ensure high-quality patient outcomes
  • P-participate in quality improvement efforts to enhance patient care
  • R-respond to patient needs with empathy and compassion
Requirements

To be successful in this role, you will need:

  • A Master's degree in Nursing from an accredited program
  • At least one year of relevant RN experience in a clinical setting
  • Excellent communication and leadership skills
  • Ability to work flexible hours, including days, evenings, nights, and weekends
