Your primary responsibility will be to ensure that our patients receive seamless care throughout their hospital stay and beyond. You'll work closely with our healthcare team to develop effective plans of care, coordinate transitions between levels of care, and provide education to patients and families on health management.

Key Responsibilities:

  • Develop and implement individualized care plans for patients
  • Coordinate transitions between levels of care, including hospital discharge and post-acute care
  • Provide education to patients and families on health management and disease prevention
  • Collaborate with the healthcare team to identify and address barriers to care
  • Monitor and evaluate patient progress, making adjustments to care plans as needed

Requirements:

  • Bachelor's degree in nursing or related field required; Master's degree preferred
  • Minimum 2 years of clinical experience in acute care setting
  • Experience in case management, utilization review, or home health care a plus
  • Certified Case Manager (CCM) certification desired
