Job Description

We are seeking an experienced Registered Nurse (RN) to join our team as a Clinical Navigator. The successful candidate will work closely with our interdisciplinary team to develop and implement patient-centered care plans that meet the unique needs of each individual.

The Clinical Navigator RN will be responsible for:

  • Conducting comprehensive assessments to identify patients' physical, emotional, and social needs;
  • Developing and implementing individualized care plans that address these needs;
  • Collaborating with multidisciplinary teams to ensure seamless coordination of care;
  • Maintaining accurate and up-to-date patient records;
  • Providing education and support to patients and their families to promote healthy behaviors and self-management skills.

Requirements

To be successful in this role, you will need:

  • An active RN license in Delaware or a Compact State;
  • A Bachelor of Science in Nursing (BSN) degree or commitment to obtain within three years;
  • At least one year of experience in Case Management, Transitional Care, or Discharge Planning in an acute care hospital setting;
  • Basic Life Support (BLS) certification;
  • Case Management Certification (CCM) within 18 months of eligibility;
  • 8 continuing education credits (CEU) per year in Utilization and/or Case Management.

Estimated Salary Range

$90,000 - $120,000 per year, based on experience and qualifications.
