The ideal candidate will possess strong communication skills and the ability to work under the direction of the RN and/or Social Worker. Responsibilities include creating and following up on electronic referrals, reviewing patient choice letters, providing follow-up messages to Medicare patients, and communicating with patients, families, and care teams.

Key qualifications include 2 years of experience in clerical or healthcare, high school diploma or equivalent, and a preferred certification as a Paramedic, EMT, or Certified Nursing Assistant. Saint Vincent Hospital is committed to compliance with federal, state, and local regulations and accreditation requirements impacting case management scope of services.
