Support Specialist for Individuals with Disabilities

Community Access Unlimited (CAU) is a social service agency dedicated to supporting individuals with intellectual and/or developmental disabilities, as well as youth within our community.
We are seeking a highly skilled and motivated individual to assist in the development and implementation of our Support Plan protocol and review processes. The ideal candidate will have a strong analytical mindset and excellent communication skills, with the ability to work independently and as part of a team.
The successful candidate will be responsible for monitoring, tracking, and reviewing Annual Individual Habilitation Plans (IHP), Individual Service Plans (ISP), and Transitional plans for youth success. They will also develop training materials, ensure compliance with licensing standards, and review members' documents and health information.
Key qualifications include experience working with individuals with developmental disabilities, knowledge of DCP&P regulations, and excellent computer and Microsoft Office skills. A high school diploma or equivalent, valid driver's license, and reliable transportation are also required.

Responsibilities:
- Monitor, track, and review assigned IHP, ISP, and Transitional plans for youth success
- Develop training materials and ensure compliance with licensing standards
- Review members' documents, health information, and other relevant data
- Ensure agency compliance with federal, state, and local regulations
- Attend Support Plan meetings and complete audits and on-site inspections
Qualifications:
- Experience working with individuals with developmental disabilities
- Knowledge of DCP&P regulations
- Excellent computer and Microsoft Office skills
- High school diploma or equivalent
- Valid driver's license and reliable transportation
Experience:
- Working with individuals with developmental disabilities: 1 year (Preferred)
- Individualized Support Plans: 1 year (Required)
- DDD Licensing: 1 year (Required)
- DCP&P Transitional Plans: 1 year (Preferred)
Job Type: Full-time
Pay: $26.44 per hour
Expected hours: 40 per week
