We are seeking a compassionate and skilled Medical Care Provider to join our team at Abode Care Partners. As a Nurse Practitioner, you will play a vital role in providing high-quality care to seniors and individuals with complex needs.

About the Role:

This position offers a competitive compensation package, including a salary range of $90,000 to $108,000 per year, plus opportunities for professional growth and development within a supportive team environment.

Responsibilities:
  • Perform evidence-based medical assessments to diagnose and treat chronic and urgent episodic illnesses, minor injuries, accidents, and other conditions
  • Collaborate with healthcare professionals to develop comprehensive care plans and ensure seamless patient care
  • Maintain accurate and up-to-date patient medical records, adhering to HIPAA and OSHA regulations
  • Provide referrals to other healthcare practitioners when necessary, ensuring patients receive optimal care
Qualifications:
  • A Master's degree in Nursing (MSN) from an accredited institution
  • An active state Nurse Practitioners license and current National Board Certification
  • Active state DEA licensure and a valid automobile insurance policy
  • At least two years of nurse practitioner experience, with a strong knowledge base of medical instruments and equipment
  • Excellent communication and interpersonal skills, with the ability to work effectively in a fast-paced environment
