Optometrist Opportunity

At Walmart, we are seeking an Independent Optometrist to lease space inside our Sam's Club locations. As an independent lease holder, you will have the autonomy to establish your days of practice, hours of operation, patient schedule, and fee schedule. You will also have the opportunity to keep 100% of exam/professional fees.

• Provide eye and vision care services to patients
• Establish and maintain a patient schedule
• Set your own fee schedule
• Keep 100% of exam/professional fees

• Doctor of Optometry (OD) from an accredited optometry school
• Valid state optometry license
• General Liability Certificate
• Professional Liability Insurance Certificate
• W-9
