Care Manager for Senior Living Communities

We are seeking a highly skilled Care Manager to join our team at American Senior Communities. As a key member of our healthcare team, you will be responsible for providing speech-language services to patients in our senior living communities.
Job DescriptionThe successful candidate will have a strong background in speech-language pathology and experience working with older adults. Your primary responsibilities will include evaluating patients, developing treatment plans, and providing individualized care to meet their unique needs.
