About the Role

Aveanna Healthcare seeks a skilled Patient Care Manager to oversee clinical operations and lead our team of registered nurses in providing high-quality patient care. As a Clinical Supervisor, you will be responsible for coordinating care, establishing care plans, and supervising patient care activities.

Key Responsibilities

  • Care Coordination: Coordinate care for assigned patients, developing goal-directed care plans and conducting comprehensive assessments of patient needs.
  • Supervision: Perform on-site visits to evaluate patient, family, environment, and caregivers, documenting findings and implementing necessary changes.
  • Leadership: Provide leadership and guidance to clinical caregivers, promoting a supportive work environment and fostering caregiver satisfaction and retention.
  • Compliance: Ensure adherence to CHAP standards, regulatory requirements, and company policies.
  • Patient Safety: Monitor and report incidents, analyzing data to identify areas for improvement and mitigate risk.
  • Nursing Updates: Provide ongoing supervision, education, and evaluation of clinical caregivers, promoting professional growth and development.
  • Quality Improvement: Contribute to quality initiatives, participating in after-hours calls and responding promptly to patient needs.

Requirements

  • Diploma, Associate, or Bachelor's degree in nursing from an accredited RN program.
  • Current RN licensure in designated state(s).
  • Valid driver's license and acceptable MVR.
  • RN experience preferred, with knowledge of private duty nursing programs.
  • Strong communication and problem-solving skills, with ability to work independently and collaboratively as part of a team.
