Care Management Opportunity

We are seeking a highly skilled RN Care Manager II to join our team at AmeriHealth Caritas Health Plan. As a RN Care Manager II, you will play a critical role in assisting members in achieving their optimal level of health by coordinating care and services.

Key Responsibilities:
• Conduct comprehensive assessments of members' needs and develop individualized care plans
• Facilitate communication between members, providers, and other stakeholders to ensure seamless care coordination
• Identify and address barriers to care, such as environmental and psycho-social factors
• Collaborate with members, families, and healthcare providers to establish and achieve short- and long-term goals
• Document progress and outcomes to ensure continuous quality improvement

Requirements:
• Bachelor's Degree in Nursing or healthcare-related field
• Current and unrestricted Registered Nurse licensure in Pennsylvania
• 3+ years of experience as a case manager, preferably in emergency department or home care settings
• Behavioral health experience, including managing care of individuals with mental health and substance use disorders
• Proficiency in MS Office, internet applications, and electronic medical record and documentation programs
• Eligible for Certification in Case Management within 12 months of accepting position
