About the Role

This full-time position involves coordinating comprehensive care plans, advocating for patient well-being, and managing resources to ensure effective, cost-efficient care. You will be responsible for assessing patients with complex health conditions, developing and implementing effective care treatments, and collaborating with healthcare teams to deliver coordinated care. Your expertise will also involve coaching and mentoring patients and their families to navigate the healthcare system and make informed health decisions.

Key Responsibilities
  • Utilize advanced clinical nursing techniques and case management practices to assess, intervene, and support patients with complex health conditions.
  • Develop and implement comprehensive care plans that address the unique needs of each patient.
  • Collaborate with interdisciplinary teams, including social workers, providers, and caregivers, to ensure holistic care.
  • Provide coaching and mentoring to patients and their families to empower them to take an active role in their healthcare.
  • Evaluate and adjust care plans as necessary to meet evolving patient needs.
  • Travel to hospitals, skilled nursing facilities, and patient homes as required to provide on-site case management and support.
Requirements

To be successful in this role, you will need:
  • 2+ years of experience in Case Management, Medical Social Work, or Brief Treatment.
  • MSW, Medical Social Work, or relevant case management experience.
Preferred Qualifications

While not required, we welcome candidates with experience in:
  • ED experience, Grief and Bereavement, Cultural Responsiveness, and Crisis Intervention.
  • Certification in Geriatric Case Management or similar.
Compensation and Benefits

We offer a competitive salary range of $120,000 - $150,000 per annum, commensurate with experience, as well as a comprehensive benefits package, including medical, dental, and vision coverage, 401(k) plan, and paid time off.
