We are seeking a skilled Physical Therapist to join our Home Health team in Olympia, WA. As a Physical Therapist with Providence, you will play a vital role in delivering high-quality patient care and improving the lives of our patients.
Key Responsibilities- Develop and implement individualized physical therapy plans to meet the unique needs of each patient.
- Conduct thorough assessments and evaluations to identify patient needs and goals.
- Collaborate with healthcare teams to ensure seamless care coordination and communication.
- Provide education and training to patients and their families on proper techniques and exercises to promote optimal recovery and independence.
- Monitor patient progress and adjust treatment plans as needed to ensure optimal outcomes.
- Current Washington Physical Therapist License
- National Provider BLS - American Heart Association certification
- National Provider Identifier (NPI) number
- 1 year of experience in Home Health or a related field
- 2 years of experience in Physical Therapy with adults with physical disabilities
- 1 year of experience working with the geriatric population
