About the Role

We are seeking a highly skilled and compassionate Registered Nurse (RN) to join our Home Health team in West Hatfield, Massachusetts. This is a fantastic opportunity for someone who wants to make a meaningful difference in the lives of patients and their families.

Job Overview

The successful candidate will be responsible for providing high-quality patient care and case management services to individuals in their homes. This role requires strong communication and interpersonal skills, as well as the ability to work independently and as part of a team.

Key Responsibilities:
  • Provide comprehensive nursing care to patients in their homes, including assessments, medication management, and wound care.
  • Develop and implement personalized care plans that meet the unique needs and goals of each patient.
  • Collaborate with other healthcare professionals to ensure seamless coordination of care.
  • Maintain accurate and detailed records of patient interactions and care provided.
Requirements

To be considered for this role, you must have:

  • A valid RN license in the Commonwealth of Massachusetts.
  • At least one year of experience in medical/surgical nursing, preferably in home health or hospice care.
  • Excellent verbal and written communication skills, as well as strong interpersonal skills.
  • Demonstrated organizational and time management skills, with the ability to prioritize tasks and manage multiple responsibilities.

Salary

The estimated salary for this position is $85,000-$110,000 per year, depending on experience and qualifications.
