We are seeking dedicated Registered Nurses (RN) and Licensed Practical Nurses (LPN) to join our team at Angels of Care Pediatric Home Health. As a key member of our care team, you will provide direct medical care to infants, children, and young adults in their homes. Job Summary:

This is a unique opportunity to work in a fast-paced environment where no two days are ever the same. You will be responsible for conducting ongoing patient assessments, administering prescribed medications, treatments, and therapies, and coordinating care with other healthcare professionals. Key Responsibilities:
  • Provide one-on-one medical care to patients in their homes, focusing on their medical needs and treatment according to the active plan of care.
  • Conduct regular patient assessments to ensure optimal health outcomes.
  • Administer prescribed medications, treatments, and therapies as ordered by physicians.
  • Collaborate with other healthcare professionals to ensure seamless care coordination.
  • Develop and implement individualized care plans to meet the unique needs of each patient.
Requirements:
  • An active RN or LPN/LVN license in good standing.
  • The ability to provide care in a client home setting, which requires flexibility and adaptability.
  • Excellent communication and interpersonal skills, with the ability to build strong relationships with patients, families, and colleagues.
