About the Role

This dynamic position requires a skilled Nurse Practitioner to provide exceptional patient care while leveraging cutting-edge technology and collaborating with a multidisciplinary care team. The ideal candidate will possess a strong passion for improving lives and communities, coupled with excellent communication skills and a keen ability to navigate complex situations.

Responsibilities
  • Deliver high-quality, patient-centered care through in-person exams and telehealth visits
  • Collaborate with Gather Guide Partners to address patients' social determinants of health
  • Communicate effectively with emergency room providers and patients to ensure seamless care
  • Document visits and clinical notes in our electronic health record system (Athena)
  • Provide on-call support for escalated cases, showcasing exceptional problem-solving skills under pressure
Requirements
  • Board Certified Nurse Practitioner with licensure in good standing in the Commonwealth of Massachusetts or the ability to obtain one prior to start date
  • Clinical expertise in evidence-based primary care, with a proven track record of delivering exceptional patient outcomes
  • Exceptional communication skills, enabling you to connect with patients from diverse backgrounds
  • Proven experience working with electronic health records and other relevant technologies/tools
  • A commitment to upholding confidentiality and navigating complex situations with sensitivity
Preferred Qualifications
  • Experience working with senior populations in underserved communities, tackling complex social challenges
  • Bilingual proficiency in Spanish, enhancing your ability to serve diverse patient populations
