Job Summary:

Catholic Health Initiatives is seeking a skilled Licensed Practical Nurse to join our Family Medicine team. As a key member of our healthcare team, you will provide high-quality patient care and support our healthcare professionals in delivering exceptional patient experiences.

Responsibilities:

  • Perform general nursing care to patients by following established clinic workflows.
  • Obtain patient consent for care and ensure patient confidentiality.
  • Provide patient care based on practice guidelines, standards of care, and federal/state laws and regulations.
  • Educate patients and families about health status, health maintenance, and management of acute and chronic conditions.
  • Document patient assessment and intervention data using established medical record forms/automated systems and documentation practices.

Requirements:

  • Graduate of an accredited Licensed Practical Nursing program.
  • Current LPN license in good standing with the state licensing board.
  • Basic Life Support (BLS) – American Heart Association.
  • Completion of IV Therapy Course.
