Care Coordinator Job Description

Harbor Health Services is seeking a skilled Care Coordinator to join our Care Team at the Elder Service Plan in Brockton. Our Elder Service Plan provides comprehensive care to over 580 participants, allowing them to live in their homes and communities. As a Care Coordinator, you will play a vital role in ensuring the seamless delivery of care services to our participants.

Key Responsibilities:
  • Collaborate with the Interdisciplinary Team (IDT) to coordinate administrative functions, ensuring compliance and quality patient care.
  • Coordinate admissions and discharges from hospitals and emergency rooms, communicating critical information to the IDT.
  • Coordinate participant admissions and discharges to and from short-term rehab and long-term care facilities, including referrals, transportation, and necessary paperwork.
  • Ensure specialty appointments have proper referrals and authorizations, and coordinate follow-up studies and appointments.
  • Document outside encounters, such as ED visits and hospitalizations, to support accurate fiscal performance analysis.
  • Ensure all participants receive care plans and associated assessments every six months, utilizing reports and Electronic Health Record investigation to ensure CMS compliance.
  • Prepare for and facilitate morning IDT meetings, including chart review, external record review, and histories.
  • Manage follow-up action items from meetings to ensure critical tasks are completed in a timely manner.
  • Coordinate multiple services for new participants, ensuring critical tasks are completed for patient safety and satisfaction.
  • Facilitate critical clinical meetings involving IDT, outside facilities, vendors, and patients/caregivers.
Requirements:
  • HS Diploma or GED
  • Bachelor's Degree in healthcare or related field (may substitute with clinical certification or Associates' Degree with experience)
  • One year of experience working in a healthcare environment performing care coordination, case management, or medical office management
  • Experience working with an elderly population preferred
  • Intermediate skill level with Microsoft Office 365, including Word, Outlook, Excel, and Microsoft Teams
  • Proven ability to resolve problems creatively, excellent verbal and written communication, and organizational skills
