Primary Care Physician Opportunity

Yale New Haven Health System is seeking an experienced primary care physician to join our team. As a primary care physician, you will be responsible for examining patients, obtaining medical histories, and treating acute and chronic illnesses.

Responsibilities

  • Examine patients and obtain medical histories
  • Treat acute and chronic illnesses
  • Order, perform, and interpret diagnostic tests
  • Involves patients and their families in care and treatment decisions
  • Create treatment plans and complete orders in patient charts
  • Collaborate on improvements in care delivery and service with staff, other physicians, and management
  • Initiate appropriate specialty referrals
  • Provide clinical information to the interdisciplinary team in a clear and timely manner
  • Ensure proper patient documentation is timely, accurate, and complete
  • Work with practice management to ensure proper compliance with billing, coding, and CPT code assignment
  • Achieve and maintain optimal patient access
  • Provide interdepartmental medical consultation as requested
  • Participate in the development of medical protocols, standard procedures, and management of patient-care-related activities
  • Provide on-call coverage
  • Give feedback to and accept feedback from all staff in a respectful manner
  • Act in a positive and professional manner, complying with practice policies, procedures, and federal and state laws
  • Perform other duties and responsibilities as assigned

Qualifications

EDUCATION

A degree of MD or DO with ABIM-recognized internal medicine residency (three years minimum) or ABFM-recognized family medicine residency (three years minimum)

EXPERIENCE

Preferred: Professional clinical experience in primary care practice, either family medicine or internal medicine

LICENSURE

Valid American Board certification in either internal medicine or family medicine and renewed (if needed) prior to expiration Valid and unrestricted Connecticut medical license BLS certification Valid DEA registration and Controlled Substance Certificate
