About the Job:

We are seeking an experienced Registered Nurse (RN) to join our team as a Home Health Nurse Specialist in Mobile, Alabama. This is a fantastic opportunity to work with a reputable healthcare organization and make a real difference in the lives of our patients.

Key Responsibilities:

As a Home Health Nurse Specialist, you will be responsible for providing high-quality nursing care to patients in their homes. Your primary duties will include:

• Conducting comprehensive assessments of patients' physical and emotional needs

• Developing and implementing individualized care plans to meet patients' unique needs

• Providing hands-on nursing care, including administering medications, wound care, and vital sign monitoring

• Educating patients and their families on self-care techniques and disease management

• Collaborating with other healthcare professionals to ensure seamless care transitions and optimal patient outcomes

Requirements:

To be successful in this role, you must have:

• A valid RN license in good standing

• At least one year of experience working as a home health nurse or in a similar setting

• BLS certification from the American Heart Association

• Excellent communication and interpersonal skills

• Ability to work independently and as part of a team
