As an Experienced Licensed Practical Nurse with Catholic Charities Family Community Services, you will play a vital role in providing high-quality care and support to individuals with mental health, alcoholism, chemical dependency, and substance use disorder related services.   • This full-time position is based in Rochester, NY, and offers a competitive salary range of $25.00 - $30.00 Hourly.">
  • You will be working in a residential setting, providing medication management, general health and wellness support, and facilitating client wellness under supervision.">
  • This role involves collaborating with other providers, maintaining confidentiality, and adhering to ethical standards.">
  • You will also be responsible for ensuring a trauma-informed care environment, effectively supporting ongoing communication among program staff, and coordinating with Residence Managers for shift coverage.">

Responsibilities:

  • ">
  • Provide medication management within the residential setting.">
  • Administer medications to clients according to authorized prescriptions.">
  • Document medication administration processes in client Medication Administration Records.">
  • Procure and log in medications received for each client.">
  • Track client compliance with required medications and report concerns to medical staff.">
  • Collaborate with Residence Managers for shift coverage.">
  • Support client wellness by observing and identifying potential medical or medication concerns.">
  • Facilitate transportation to necessary medical appointments and promote health and wellness within the facility.">
  • Assist Medical Staff and Prescribers as needed.">
  • Participate in staff meetings, medical/clinical supervision, and relevant Agency meetings/trainings.">

Requirements:

  • A minimum of a High School Diploma or GED and accredited LPN diploma is required.">
  • A valid Licensed Practical Nurse credential is mandatory.">
  • Two to three years of experience in nursing services to individuals with mental health, alcoholism, chemical dependency, or substance use disorder related services is preferred.">
  • Bilingual proficiency in Spanish is a plus.">
