Job Description
As a Hospice Case Manager Nurse with Amedisys Inc, you will have the opportunity to provide, coordinate, and direct the provision of hospice nursing and palliative care to terminally ill patients in their homes and communities. You will work closely with attending physicians, patients, and their families, as well as other members of the patient care team, to ensure continuity of patient care and achieve excellent pain and symptom management and high-quality end-of-life care.
Responsibilities
- Perform patient assessments and collaborate with the care team to develop and implement a plan of care.
- Makes referrals to other hospice care disciplines as needed or documents rationale for not doing so.
- Develops, prepares, and maintains individualized patient care progress records with accuracy, timeliness, and according to care center policies.
- Records pain/symptom management changes/outcomes as appropriate.
- Teaches patients, families, and caregivers appropriate patient care techniques.
- Regularly communicates with clinical manager and care team.
- Performs on-call responsibilities and on-call services to patients/families as assigned.
Qualifications
To be successful in this role, you should have 1+ year(s) of nursing experience in acute care settings, ER or fast-paced environments. You must also hold a current RN license specific to the state(s) you are assigned to work, as well as current CPR certification and a valid driver's license with reliable transportation and liability insurance.
