Job Summary

Catholic Charities Brooklyn and Queens is seeking a skilled Clinician to join our team. As a Clinician, you will play a critical role in providing high-quality healthcare services to individuals in need. Your primary responsibility will be to coordinate care for clients with complex medical and psychiatric conditions, ensuring they receive the necessary medical, behavioral health, and social services to achieve optimal health outcomes.

Key Responsibilities:

  • Conduct comprehensive psychosocial assessments to determine client appropriateness for enrollment and identify medical, psychiatric, housing, and other social needs.
  • Develop and implement individualized care plans that integrate medical, behavioral health, rehabilitative, and social service needs.
  • Provide intensive emotional and practical support to clients as they transition back into their communities and into supportive housing living environments.
  • Collaborate with community providers to ensure clients receive connected services and share or collect collateral information.
  • Engage clients while inpatient and post-discharge, conducting home and community visits to promote independent community living and optimum mental and emotional health.

Requirements:

  • Master's degree in social work, mental health counseling, or marriage and family therapy.
  • New York State Licensed Master Social Worker or Licensed Mental Health Counselor required.
  • Minimum of 2 years post-masters work experience in the mental health field.
  • Knowledge of mental illness, serious emotional disturbances, and substance abuse disorders.
  • Ability to develop, evaluate, implement, and modify clinical treatment interventions to meet individual client needs.
