Covenant Health is seeking a highly skilled and compassionate RN Case Manager (Non-Certified) to join our Transitional Care Unit team. As a key member of our multidisciplinary team, you will play a vital role in ensuring the highest quality care for our geriatric patients.

About the Role

The RN Case Manager (Non-Certified) is responsible for coordinating patient care continuity and quality through the collaborative development of practice guidelines and clinical pathways that support quality improvement activities. You will work closely with physicians, nurses, and other healthcare professionals to promote patient-centered care, assess patient needs, and develop comprehensive care plans that address individualized goals and objectives.

Responsibilities
  • Utilize case finding criteria to screen patients and gather information from medical records, physician documentation, and patient/family communication to develop a comprehensive plan for each patient.
  • Assess and periodically reassess patient progress towards established goals, modifying the case management plan as needed to meet changing patient needs.
  • Collaborate with the healthcare team to design and implement practice guidelines and clinical care designs for assigned populations, securing necessary resources to ensure timely, efficient, and cost-effective services.
  • Communicate effectively with physicians, patients, families, and other healthcare professionals to plan and implement patient care, documenting communication results and directions in the patient's medical record.
Requirements

To be successful in this role, you must have:

  • A minimum of two years of acute care nursing experience, preferably in an assigned area of responsibility.
  • A valid Tennessee State RN License and a commitment to maintaining ongoing fiscal awareness by communicating outcomes quarterly to the team.
  • An ability to identify causes of outcome variances, implement actions to improve them, and evaluate corrective actions for improvement.

Covenant Health offers a competitive salary range of $80,000 - $110,000 per year, depending on experience, plus benefits such as health insurance, retirement savings, and paid time off.
