Job Summary

We are seeking a skilled Occupational Therapist to join our team at Community Hospice of Northeast Florida. As an Occupational Therapist, you will play a vital role in evaluating and treating patients to improve their level of function and prevent disability.

Key Responsibilities
  • Evaluate patients' level of function and develop treatment plans to improve their level of function or prevent disability
  • Conduct initial therapy assessments and periodic reassessments, documenting and reporting changes in patients' conditions
  • Recommend treatment plans and coordinate with supervisors and interdisciplinary teams
  • Communicate with patients, families, and healthcare teams to ensure effective care
  • Participate in educational programs and maintain current and accurate records of evaluations, treatment data, and patient responses
Requirements
  • Current Florida Occupational Therapy license
  • Current Florida driver's license with proof of active auto insurance
  • Current CPR certification
  • Graduated from an occupational therapist education program accredited by the Accreditation Council for Occupational Therapy Education (ACOTE)
  • Eligible to take or has successfully completed the entry-level certification exam for occupational therapists developed and administered by the National Board for Certification in Occupational Therapy Inc. (NBCOT)
Preferred Qualifications
  • Minimum of 1-year working as an Occupational Therapist
  • Minimum of 1-year working with a frail or elderly population
  • Experience working with Alzheimer's and people with dementia
