Job Description:

We are seeking a skilled Home Health RN to join our team at HCA Healthcare. As a key member of our interdisciplinary team, you will be responsible for providing coordinated skilled nursing care to patients in the comfort of their own homes.

You will work closely with patients, families, and caregivers to establish and achieve personalized goals, ensuring high-quality patient care. Your duties will include assessing patients' physical, psychosocial, and environmental needs, completing OASIS assessments, and communicating significant findings to the clinical manager and physician.

Key Responsibilities:

  • Assess home care patients identifying physical, psychosocial, and environmental needs
  • Complete OASIS, assessment, and visit paperwork according to agency policy
  • Communicate significant findings, problems, and changes to Clinical Manager and physician
  • Supervise and provide clinical direction to home health aides and LPNs/LVNs
  • Participate in on-call rotation and emergency call according to agency policy

Required Skills and Qualifications:

  • Graduate of an accredited school of professional nursing
  • Minimum of one or more years of home health, public nursing, or acute hospital nursing experience
  • Familiarity with Medicare home health regulations, documentation requirements, ICD-10 coding, and PPS
  • Reliable transportation and proof of valid automobile liability insurance
  • Current BCLS Certification prior to providing patient care
